How does a patient obtain financial assistance?

Is FACT a United Way Agency?

As an Associate United Way Agency, FACT receives only “donations designated” by the contributor on one’s pledge card. Residents outside of the county can donate to FACT by designating their contribution to “Tiffin-Seneca United Way-FACT” on their pledge card, tax ID# 34-1658459.

Who is eligible to get help from FACT?

There are two criteria for FACT eligibility, you must live in Seneca County, Ohio and have a cancer diagnosis.

Can anyone help a cancer patient apply for services from FACT?

Yes, application can be made by one of the following referrals: Patient, family member, physician, hospital, or other community agency.

Is all of my medical information keep confidential?

Yes, all names and information will be treated with confidentiality by FACT.
