How much of the money contributed to FACT remains in Seneca County of Ohio?

How does a patient obtain financial assistance?
Call us at 419.937.2540 or download our application and physician authorization form located on the homepage. Once you've filled it out, send it to FACT at P.O. Box 624 Tiffin, Ohio 44883.

Is FACT a United Way Agency?
As an Associate United Way Agency, FACT receives only “donations designated” by the contributor on one’s pledge card.  Residents outside of the Fostoria or outside of the county can donate to FACT by designating as follows: “Tiffin-Seneca United Way-FACT” on their pledge card.

Who is eligible to get help from FACT?
There are two criteria for FACT eligibility; you must live in Seneca County of Ohio and have a cancer diagnosis.

Can anyone help a cancer patient apply for services from FACT?
Yes, application can be made by one of the following referrals: Patient, family member, physician, hospital, or other community agency.

Is all of my medical information keep confidential?
Yes, all names and information will be treated with confidentiality by FACT.
